An important change in the law about absentee voting. Effective January 1, 2022, an eligible voter may not cast a ballot on a machine at a poll site after the Board sends an absentee ballot to such voter following a timely request. Regardless of whether the absentee ballot was actually delivered by the postal service or was sent back to the Board, the voter will not be able to vote on a machine at a poll site. The voter may still complete an Affidavit Ballot at the poll site.

Qualifications to Register to Vote in Suffolk County
  • be a United States citizen;
  • be 18 years old by December 31 of the year in which you file this form (note: you must be 18 years old by the date of the general, primary or other election in which you want to vote);
  • resident of this state and the county, city or village for at least 30 days before the election;
  • not be in prison or on parole for a felony conviction (unless parolee pardoned or restored rights of citizenship);
  • not be adjudged mentally incompetent by a court;
  • not claim the right to vote elsewhere.

How and Where to Register to Vote in Suffolk County

  • You can register in person at your county board of elections or at any New York State Agency-Based voter registration center.
  • You may also submit your voter application form at the Department of Motor Vehicles, either in person or on their web site if you already have DMV-issued identification.
  • You can pick up a New York State Voter Registration form at your local library and mail it back to the Suffolk County Board of Elections.
  • You can call the 1-800-FOR-VOTE hotline to request a voter application.

You can complete a PDF version of the New York State Voter Registration Form on-line by clicking on the link below, typing the necessary information and selecting the appropriate boxes. Alternatively, you can print the form to complete by hand.

Once the form is completed, you need to print the form and sign it. The Board of Elections will not accept any kind of digital or Adobe-generated signature. Then, mail the form to the Suffolk County Board of Elections.

You can also complete an Accessible version of the New York State Voter Registration Form on-line by clicking on the link below, typing the necessary information and selecting the appropriate boxes.

Once the form is completed, you need to print the form and sign and date it. Then, mail the form to the Suffolk County Board of Elections.
